CA Breast metastasis to left cerebellum

New Case 48 yr old ,F, Had undergone Radical mastectomy in 2018,and chemo and regular follow up in RCC.Asymptomatic till 10 days ago when she developed vertigo with intermittent vomiting. Evaluated by neurologist and started on vertin. Took vertin for a week but no relief in vertigo and hence attended the Neurology opd. On exam Bp130/ 80 mmhg ,normal optic fundi and the only abnormality is left sided early cerebellar signs with difficulty in tandom walking An urgent MRI brain was done and ref the case to Neurosurgeon and surgery was done today as an emergency case. Diagnosis?

Thanks Curofy and all Doctors who answered the case. MRI report: Moderate sized nearly globular SOL in the central part of left cerebellar hemisphere, having moderate enhancement. The entire left cerebellar hemisphere and superior vermis shows diffuse swelling.The superior vermis is having minimal midline shift. MRI reveals high perfusion on the lesion. Impression. Metastasis vs high grade glioma. Surgery done.

looks like a malignant lesion , either a metastatic deposit or new primary tumour , get ct chest and abdomen to rule other metastic deposits, mean while give her dexamethsone 8 mg 8th hourly to reduce brainstem edema, and decongestive therapy.
