a 54 yrs female came with compliant of left side lower abdomen pain and nausea,k/c/ BAand renal calculs,no history of fever and chills, Rx given drotin 1amp tab.zenflox UTI bd tab.meftal spaz tds tab.pan 40 od tab.deflocart bd tab.ultrace (sos) with emeset review after 3 days. invistigation urine routine and USG kub she came back same pain with reports urine routine is looking normal ? further rx

The diagnosis is still not established. Hydronephrosis is just a finding. Aetiology must be found. You need a CT kub plain. One possibility is slow ureteric/vuj calculus. Other possibility is a passed calculus with residual changes in the pelvicalyceal system. The only drug that works for renal colic is diclofenac. You could also add a mild diuretic like dutor 5mg once a day and plenty of fluids. Definitive treatment will depend upon the ct report.
