New itchy rash on left arm

A 63 year old female with a history of prediabetes, obesity, hypothyroidism, bipolar disorder, chronic pain in knees on levothyroxine, carvedilol, gabapentin, meloxicam, ability, benztropine. Seen for a new itchy rash on her left arm only. It has Been there for 3 weeks. No improvement with 0.1% triamcinolone cream, states it burns with steroid cream. She has been trying Benadryl cream for some relief. She is able to shower and take care of her own hygiene. Other family members do not have this rash. What is your opinion?
