60yrs old male known diabetic and Hypertensive off drugs since last 3yrs today found unconscious in ground with snoring respiration and decerebrate rigidity.Intubated and mechanically ventilated upon arrival.GCS - 3/15,BP -200/100,Pupils - B/l Dilated fixed non reactive.WHAT APPROACH SHOULD BE TAKEN?

Left caosuloganglionic bleed with uncal herniation to rt with severe brain edema,compression of left lateral ventricle, sweapage of blood to brainstem, lateral, 3rd and 4th ventricle with mild dialatation of rt lateral ventricle. Suggest Active conservative nanagemwnt.

A massive intracranial hypertensive bleed left parietal region with left lateral ventricle extension with global ischemia, gross midline shift also brain stem involvement. An urgent decompresive craniectomy is the only hope of some survival. Also gradually bring down the B.P with labetalol in drip and maintain strict glycemic control with insulin besides AED'S . Prognosis is poor
