Abdominal pain ,vomiting and malena in a 11 year old boy . Abdominal ultrasound shows enterocolitis and medical renal disease ( hyperechogenicity of renal cortex) DD Henoch Shonlein Purpura Enterocolitis , with sepsis, intussusception, polyposis
Black stools Usg suggest enterocolitis Likely a c/o bacillary dysentery Send stool for routine and c&s Treat iv antibiotics like amikamicin Inj meteronidazole And rest of supportive treatment
USG shows enterocolitis NPM Ryles suction IV Fluid Inj Metronidazole 10mg/kg 8 hourly Inj Amikacin 15mg/kg/day in 2 divided doses Inj Ondansetron 4mg iv bd Inj Drotavarin 40mg im bd After patient is stable CBC, Electrolytes, Upper GI endoscopy, Colonoscopy Stool for RE with OBT Treat as per reports
USG finding enterocolitis which nothing but a bacterial infection manifested by loose stool mixed with blood or black tarry stool mixed with mucous Need clinically evaluation by CT abdomen Stool for examination and culture and sensitivity
Look for occult blood in stool. Treat it as a case of enterocolitis give tab lcin oz Probiotics tab drotin for pain ondasetron for emails.
