Its a slipped capital femoral epiphysis. Kindly do an MRI to assess the vascularity of the head and the CAM impingement. Get an endocrine opinion and also rule out hypothyroidism. It's a chronic slip. It needs a bilateral surgery . Tackle the right first by doing a safe surgical dislocation and bumpectomy, and debridement of the acetabular labrum. Once he is well do the same to the left. It needs to be tackled by an expert only.
