By following the anatomy of tooth, anatomical landmark in the vicinity and direction of the embossed dot which is towards the occlusal surface facing the direction of exposure.

To differentiate upper and lower x-ray u should know or follow the anatomical landmarks of maxilla and mandible. And for right and left, follow the dot or bump present on the x-ray film and it should always towards the occulsal surface which is facing the direction of exposure. Or simply hold the film in front of u and just see how teeth are present like it always present anteriorly towards posteriorly. According u will get to know the side.
