Skin lesion since last 6 months

45 y/M c/o this kind of skin lesion on his left mid back,severe itching and serous+ blood discharge from it while itching.give ur valuable opininion about Dx,ddx and Rx

Vesicular lesions puncted with itching In a dermatome Most likely HERPES ZOSTER with herpetic dermititis Rx acyclovir 800mg 5times a day for 7days and tapper off gradually Tab azithromycin 500mg 1od Tab Levocetrizine5mg 1bd Tab gabapentin 100mg+tab nortryptiline 10mg 1od at bedtime Locally acyclovir oint 5times a day Review every fortnightly
