16years old female c/o pain abdomen sudden onset localised at supra pubic n right iliac region , burning n painful urination since 4days No history of fever.. Has vomiting sensation.. Kindly let me kno wat would b d further intervention n management.. Thanku

young female with acute abdominal pain in lower abdomen. . DD S. . 1. acute appendicitis. .but without fever. .vomiting. tachycardia. no probe or mcburneys tenderness. .It is unlikely. . 2. cystitis. 3..mesenteric adenitis 4. uvj calculus obstruction. . 5.meckels diverticulitis 6.chrons disease 7.DKA 8. If menarche not attained...think of menarche or haematocolpus. 9. If attained menarche. .unwanted pregnancy and ectopic rupture 10. haemorrhage into ovarian cyst.. 11.if pain in the left iliac fossa..think of appendix epiploica. .

Urine examination shows plenty of pus cells. It indicates urinary tract infection. Please do culture urine first and start antibiotics afterwards.Do haemogram . See clinically signs of appendicitis. In pelvic appendicitis if appendix is in contact with bladder frequency of micturition may be seen. So first of all think of UTI. If you want to know whether it appendicitis then go for MRI to finalize the dx.

Definitely there is UTI, she needs antibiotics for that What are the examination findings? Get a surgical opinion to rule out appendicitis And all of jumping on appendicitis by looking at USF and small history makes me anxious It's like jumping the gun

Advice for surgical management.... In ayurveda we can't cure this patient completely....

treatment with iv fluids and antibiotics ..All the investigations to rule out the above causes. .

This is clearly acute appendicitis. do A HEMOGRAM, & CHECKED TLC. IF TLC IS MORE THAN 15000/CMM, THEN GO FOR SURGERY, OTHERWISE U CAN MANAGE HIM CONSERVATIVELY.

Ayurveda has better management fr uti. uti shld b addressed. bruhatyadi kashaya, chandraprabha vati is a nice combination. along with that ardha bilwadi paniya kalpana. clinical confirmation of appendicitis shld b considered fr further management. avipattikara churna dhanwantara vati with sukumar kashaya can b considered
