Looks,like a case of viral fever. Widal test is inconclusive and it has no value - if done before 7 days of onset of fever . Get Salmonella typhi antibodies IgG and IgM done . Also get urine culture done as urine R/E shows 5- 6 pus cells . Till the report of urine culture and tyhoid test comes - just give symptomatic treatment with Paracetamol 650 mg × hourly Cold sponging S.O.S Multivitamins.. Appropriate antibiotic only of urine culture and typhoid test is positive
even after repeated discussion . again widal test was done in 2 nd day of fever . it should only b done after 7 days of fever . remember the code BASU . its being taught during mbbs days . here crp is positive n pus cells in urine . send urine c/s . treat it as uti . start levofloxacin 500 mg once daily .
Blood reports show leucocytosis ,poly predominance, raised CRP and normal PLT. Infective fever, bacterial ,? Viral Suggest serial CBC, daily clinical examination for emerging clinical picture. Blood and Urine culture, xray chest and abdominal ultrasound..
CRP high and fever 102°F Needs chest X-RAY PA view. DENGUE Test. Urine C&S. Tab. Azithromycin 500 mg OD X 5 days. Tab. Ofloxacin 200 mg X BDPC X5 days. Tab. Pantoprazole 40 mg OD X BBF X 5 days. VIT- B complex. 10 ml BDPC. Tab. Paracetamol 650 mg 1tab. 6 -8 hourly. Electral Or ORSL continue. Balance dietary supplements and rest.
Kindly look for Enteric fever confirm by S typhi IGM Covid 19 by RTPCR Simple VF Dengue by dengue serology
Look like viral fever as CRP raised Symptomatic treatment /basic On 5th day dengue ns1 igm igi Close watch for other symptoms
