a 4 yrs old boy presented with c/o wound on both hip leg and face( see pic) for one and half years. please suggest investigation diagnosis and treatment.

Pyoderma / impetigo contagiosum
In my opinion it is nodular scabies with secondary infections not treated specifically and hence such a long period yes due to scratches at places it appears to be impetigo but primarily i will treat as scabies. Daily bath with zocon soap bar Orally tab or syp azithromycin 100mg od for 5 days or clindamycin 150mg 1bd till infection is resolved Syp Levocetrizine5mg od for 5to7 days Local application of permithrine oint 5%30gm to be applied all over body at a time and to be repeated after one week Local fucibact oint on lesions having infection Review after one week

Impetigo contagiosum Pyoderma Azithromycin 250 mg OD and levocetirizine 5mg OD for 7 days after 7 days CV 375mg OD and atarax syrup OD for next 7 days and mupirocin ointment twice daily for 10 days

Impetigo contagiosum. Local application of betadine. Broad spectrum antibiotics orally. Can give combination of amoxicillin and Cloxacillin.
