21 years old Saudi female with dark skin, medically fit, came complaining of a large swelling in the left palate that bothers her during speaking. History of C.C: Painless mass slowly growing over the past 1 year. Clinical examination: Extra-oral mass in the left cheek. Intra-oral examination revealed painless fibrous to hard mass extending from the left soft palate until tooth #23, and up to the midline of the palate. Posterior teeth were displaced. OPG radiograph showed radiolucent lesion in the posterior left maxilla with severe apical displacement of tooth #27. A biopsy has already been done and the histopathological results have been obtained. My question is can you give your differential diagnosis for this case? What is the treatment of your differential?

Yes! The 3rd differenitial is the one, but can you think of other types that give this fibrous consistency and in a young age.

Very interesting case.. thank you for sharing. DDx: - Myxoma - Ameloblastoma - KOT Tx: - Take CBCT for further evaluation. - Surgical resection.

Thank you for passing by, Malak! Well, a CBCT has been taken already but couldn't share it. A very nice DDx! It is the 2nd one, but with this consistency and in such young age what do you think the type of ameloblastoma she has?

squanous cell carcinoma...appears like sarmatoid variant.....other DD are sarcoma

Not really. Think about aggressive benign tumores.

It could be pleomorphic adenoma Can you share with us the definitive diagnosis?

It is not eman. Not a slaivary tumore at all. Think more towrds a very common expansile tumore in KSA that comes in fibrous form. Very easy!

It is benign as mentioned, because it is pushing the vital structures and teeth (also root) rather than invading. Can't wait to see the diagnosis. Thank you for sharing this interesting case.

Its a case of polymorphic adenoma.

If possible can you share the histological findings? Interesting case thank you.

Sure thing.
