21 yom. PMH-Onset sore throat, fever 102.3, vomiting approx 30 hours prior. Rapid Strep swab negative. Patient has had intermittent episodes of Strep since childhood. How often are Rapid strep swabs inaccurate? Differential?

It is not sore throat but a c/o fulminating tonsilitis . Best adv was to go for tonsillectomy since childhood pt recurring bcz of harbouring of infection Strep test may or maynot be +ve hence decision is to be on clinical findings

Pic in not clear but an inflammation and grade 4 tonsils can be seen may be a case of acute exacerbation of chronic tonsillitis. Swab may not always accurate. If swab is +ve we can conform the organism but a -ve swab expecially from oral cavity will not rule out infection. A repeat swab can be taken

Infectious mononucleosis If its not resolving since long than this can be one provisional diagnosis Other than that recurrent tonsillitis but swab test was negative as you told. Associated with fever,so can be viral.
