A known complication of RA i.e protrusio acetabuli. If he is symptomatic then only option is THR. Either cemented with a cage or uncemented with milled head as bone graft into the acetabulum. If asymptomatic then bone strengthening drugs and hip exercises.

23 male rhumatiod arthritis and back pain. Quite rare. RA usually seen in middle age in females. Also back pain is very rear in RA. Looks more like AS. Common in young males. Do HLA-B27 if possible. Look for evidence of uveitis. X-ray shoes protrusio acetabuli. Seen with AS, RA, marfans, osteomalacia. Management is ideally surgical.

THR if patient is symptomatic. not common to see 23/m RA

whats about ADL.? whether b it RA /AS - only definite way is THA but should b deffered as long possible with conservative management.

Total hip replacement is the only option.

Only treatment is THR but it's better to defer it for sometime if patient doesn't have much problem.

Treat RA on medication and pt will require Uncemented THR keep allograft,mesh ready for acetabulum. Do one hip at a time
