Pleural effusion

24 yr male presented with fever cough SOB and Rt side chest pain since 20 days. Xray chest was ordered. Pleural effusion was found. Pleural fluid was sent for further investigations. Report attached. Should I start ATT or need some further supportive investigations?

Rt sided pleural effusion with fissural extension noted. Routine fluid analysis sugg of Koch's etiology. Repeat fresh sample for ADA. Consider AKT.

Commonest cause of pleural effusion is TB. Except normal ADA , all results support TB. Pleural aspirate AFB CBNATT will help.clinch diagnosis. Can start patient on ATT with the present evidence.

Sir.... It may need mountex test and better to start ATT with higher antibiotics.... but better to evaluate HRCT....
