Health care delivery during Lockdown?

This is a serious discussion with all curofy doctors considering the lockdown all over the country. Right now medical college hospitals are concentrating on Emergency services and COVID- 19 related activities only . Most of the private O.P.D 's are also closed . There are severe restrictions in Lockdown and movements of common man are restricted Some cases are emergencies which will have to go to hospitals- no options. People are facing serious problems regarding semi emergencies and O.P.D level medical diseases which they are now self treating or at the most taking time burying medications from chemists .What is the solution to this problem. If some one is having a doctor as a relative - they can take consultations and guidelines telephonically. What about common man .How they will be treated. Is not telemedicine a viable option. Govt. as well as our medical fraternity should think over it

Under the present circumstances, there are limited options for patients to get doctors opinion for their illnesses. Consultation through phone and WhatsApp seems to a reasonable option . Telemedicine may not be a perfect or permanent solution, but it can do valuable service in the coming 2 months of Covid related restrictions . I have found that a reasonable working diagnosis can be arrived in most of the telemedicine cases ,and appropriate treatment prescribed . In case of emergencies ,like chest pain or stroke, advise can be given to the relatives for urgent hospital admission . We can do an excellent job with telemedicine ,to known patients . So telemedicine can be restricted to patients of our own practice . If we have to do consultations for unknown patients we need to spend more time, getting to know them . We can do this free ,considering this service as an opportunity for us to pay back what we have received from the community. Even otherwise we can receive payment digitally .

Sir you have rightly pointed out the problems facing by the common people who are affected with non Corona problems as all private practicing doctors have restrain themselves for their Chambers and opds of most hospitals also closed their activities. Really it tough time for the semi emergency cases who need medical assistance. But this is also a fact that if you want to run the outdoors as usual there will be rush if patients which will go against the social distance rather encouraging the violation of lock down norms. I can say in my mother institute there is a complete outdoor complex where all the departmental opd used to run and approx 8000 patients and their relative used to gather at our time in late sixties and early seventies,now due specialisation no of opd increased and no of patients also increased.It is meaningless to impose lock down and run opds simontaneously. So for the greater interest these things have to be sacrificed.

In principle telemedicine is good concept But it has lot of limitations To dx on pictures and story posted is erroneous and may differ from physical examination and thus line of treatment will differ 2 without supporting investigations to conclude on dx is always a challenge 3 accountability of adviser paramounts 4 not necessarily all patients will post what they are facing actually and what is to hide 5 pt talks of their interest only while cross questioning on history we come to know what is actual direction like dehydration pt will not able to express in telemedicine except a term weakness So there are many mor reasons Last but not least all consultation will go free without appropriating your efforts These are my personal considrrations

In our state of Odisha, IMA has taken an initiative to consult doctor volunteers via tele medicine. They have given their specialised field, mobile number with time of consultation and the local IMA branches published it & circulated on social media.

Telephonic consultation is best at this point considering the self distancing is not maintained once opd is open.We become main reason for community spread here if we don't close down

Semi emergencies should also be made. O P D can also be maintained with preventive measures of corona like handwashing, distance between two persons, mask etc

- all govt. Sector starting from PHC, CHC, Cottage hospital , District Hospital, Civil hospitals are not under lockdown.
