Chest Pain.
28 year old male.Recently diagnosed case of hypertension not taking medication.Severe chest pain.BP 160/90 mm hg.
ECG shows NSR, sinus tachycardia.. limb leads show swaying baseline... could not comment on ST-T changes... Young hypertensives.. needs to be given Betablocker as first line drug. Get urinary VMA to ruleout Pheochromocytoma.

ECG NSR with tachycardia Tb ivabrad 5 mg BD. Tb telma AM 40/5 mg OD Tab anxit .5 mg HS Adv KFT,lipid profile,fundus examination, HbA1c and USG KUB region.salt restricted diet
Sinus Tachycardia LVH by Sokolov Criteria Lower BP by i/v medication not by >20% in one hour.
S.Tachy LVH Advise Echo to ruled out stractural heart dieases
ECG shows sinus tachycardia Most possibly this hypertension and sinus tachycardia due to chronic stress.

a male paitient of 20 yrs age is facing high bp problem. his bp reaches from 210/140 uotp 230/150 . he is facing this problem from past many years. undergone with all the medical checkup like lft, rft, sugar, thyroid, heart diseases, cholestrol . all the reports a normal except cholestrol that came 226 in recent checkup. he also suffers from a problem of blood coming out of his nose in gap of 4 or 5 months or off a sudden in couple of days. generally he do go through the head swims but he tackle it normally. even his cbc is in normal level, now he is taking medicine of high bp only i.e telgon C and a medivine for cholestrol. but still he is facing bp problem and nasal blood flowing. Doctors are helpless in diagnosing .if any one of you have any expected diagnosis for him then please let me know. NOW THE PAITIENT IS SUFFERING CHEST PAIN IN RIGHT SIDE. AND ALSO HE IS SUFFERING PAIN IN UMBLICUS LEVEL BELOW STOMACH REGION. PLEASE HELP HIM TO RECOVER.
