It's nice finding On second look, out does look like pulmonary embolism with S1Q3T3 2 D ECHO is correct choice of investigation

N axis NSR S Tachycardia If he recovered from COVID then his last CXR & D-dimer would have been normal on discharge , so very little chances of pul embolism. Post COVID anxiety and depression is very very common. So in my opinion you treat in that way only.

I am agree with @Dr. Sandeep Ghodekar . And essential take some rest of physical and mental both.

Dr Sandeep is appreciable to pick up finding of s1q3t3 Proceed with levels of d dimer and heparinize the patient
