There is a limit for space and hence ,not included everything. Cont MRI gyral edema left temporal CSF 400RBCs,30 WBCs lymphocytes ,sugar 70 mg/ dl,protein 60mg / dl.What is the most causative agent?
HSV 1
Temporal lobe gyri edema seen in herpes simplex encephalitis . HSV PCR in CSF can help in diagnosis.

A 32 y/o female with a history of hypothyroidism and no history of seizures presented with an acute onset of behavioral changes and witnessed seizure activity. 40 yr old male symptomatic for 5days.Last sunday while standing for prayer he suddenly felt the sensation of some one pushing him forward.This unusual feeling he had informed to his colleague .After an hr, while travelling in the scooter developed subjective vertigo & stopped the vehicle& felt better with in 30mts.Drew the scooter for 30mte, reached home & vomited.Felt better, took lime juice & slept.4am on the next day while washing the face again developed vertigo ,sat down& then be ame unconcious The wife noted her husband lying down in thence dischargefhe floor with nodding movements of the head& neck with some clonic movements in the upper limbs lasted for 3 mts.After 5 mts he regained the conciousnes& could recollect every thing till he sat down in the floor.He attended hospital & examination failed to obtain any deficits & hence discharged.After an hr at home again developed one more episode of vertigo followed by the same type of previous involuntary movements lasted for 5mts Re-evaluated at the same hospital & diagnosed to have sleep deprivation related tension.He wal alright during the rest of the days.The next dat ie Tuesday when he got up in the morning the wife noted some tremors in the hands with difficulty to hold the cup of tea.Admitted at the same hospital. Speech was clear & blood work up Mri brain& Eeg were requested..Blood work up were reported as normal EEg was also normal& MRi brain was done which was reported to have stroke.PT was in the ICU.The next day ie wednesday the wife noted defective clarity of speech with unsteadiness with tremors in the body.No fever/ headache. Thursday' morning developed breathing difficulty? seizure ,intubated and ventilated.Thursday mid night the pt was transferred to our hospital.On exam,on ventilator, temperature was 100.6deg f.Normal optic fundi & pupils.Deeply unconcious probably due to sedation also.Bilaterally hyperactive DTRS with extensor plantars.Chest, abdomen & heart -normal.nNo past h/o of medical illness. All blood work up including fever work up -normal.CSF-clear,8 cells1poly,7 lymph.Sugar88mg/dl.,protein 34 mg/dl No organism.EEG&MRI BRAIN ENCLOSED. GIVE OPINION OF THE CASE.CURRENTLY PT IS IN THE MDICU WITH APPROPRATE MED.
