we had a conference today where different professors from Moscow And St. Petersburg participated. Highlights: 1) desloratadine for urticaria, latest developments state that it dosnt just act as anti H1 but also as anti inflammatory by inhibiting many pro inflammatory interleukins and other cyto and chemokines, so can be used not just in urticaria but in other allergic inflammatory conditions. 2) apremilast is a promising anti psoriatic drug. in a study number of patients who failed to respond significantly to biological treatment (anti TNF) improved well with apremilast. as the drug is taken orally, the compliance rate was much higher. 3) Rosasea. the Role of seborhea is excluded from pathogenesis of Rosasea. shockingly on contrary to general belief that ophthalmorosasea is a rare entity, in recent large study (dont remember the article name) the frequency of both papulopustular and ophthalmorosasea was found to be same at 36%. and treatment of Rosasea with ivermectin topical for 16 wks was superior to all other forms of topical treatment in papulopustular Rosasea and effect from treatment was evident in 2 wks itself from start of treatment. also Few words about new drug Brimonidin 0.33%gel which is the only topical preparation approved by FDA for erythematous teleangiectatic type of Rosasea. 4) secukinumab (anti IL 17A) in psoriasis was compatred qith other biologics including ustekinumab (anti IL 12, 23). the response was better to other biologics and noticeable percent of patients achived PASI100 clearance. but most importantly the recent increasing issue with tachiphylaxis to biologics was not noted in case of Secukinumab, and there qas statement from professor that some study found antibodies against it but which were natural meaning the tachiphylaxis will never develop against Secukinumab. 5) New treatment for Molluscum Contagiosum with 5% KOH applied twice daily until a noticable inflammatory response surrounding these lesions is induced and then stopped. results were fantastic. pictures are not available, sorry. 6) a very simple yet commonly mistaken clarification was made that many Doctors put diagnosis of furuncle in paediatric patients below age 2 years. He said its ALMOST impossible as the substrate (follicle) itself is not completely developed until 2 years of age. 7) onychomadesis due to HFMD was presented. will be having second session tomorrow. i will update more after my exams tomorrow.
