54year female complains of loose stools ..vomiting since morning h/o fever..headache no h/o chest pain..breathlessness k/c/o DM on regular treatment PR..110/min BP...110/70...RBS..229 SYSTEMIC EXAMINATIONS...NAD Sir/Madam kindly give your valuable opinion for further evaluation and treatment..

8 year male child presented with complaints of high grade with chills x 5 day (103 to 105) with abdominal pain with no rash , burning micturition.o/exam child had epigastric and subcostal region tenderness + , spleen + 2 cm , liver 3 cm bcm. chest clear , cns , CVS nad., no lap. with normal sensorium.. child investigation CBC {5th day of fever - CBC normal , lft wnl , with ns1 negative. usg and - s/o multiple mesmeric lymph node largest of 20 mm. with hepatosplenomegaly.. Widal - to-1/320, th -1/160. esr -38..repeat after 4 day esr - 10. platelet 1.20lac. tlc 5000. hb 10. montoux negative p.s mp -ve ldh 1250 . ..received tab cefixime and azee x 3 day tab lumerax (artether with lumifantrine ).with not much improvement started inj mono cef at 75mg/kg , inj amika with azee since last 2 day. fever is still coming but only during night with chills..with complaints of vomiting and loose stool with spotting of blood. blood culture awaited. please advice possible line of treatment or did regards

A 8 years old boy who present with a history of 2days abdominal pain which aggravated from that day evening. There was little breathlessness from that evening onward.No history of fever or vomiting or loose stool were there. On examination the patient seen anxious, there were tachycardia with tachypnea. Spo2 97%, left side full diminished breath sound, with normal temperature. Abdomen seen little rigid with mild upper left quadrant tenderness. There was no significant past medical or surgical history, but the parents informed he having some stomach pain on and off for last 3months for which they frequently visited paediatric OPD.

A 11 years old boy c/o fever, head ache and cough. Fever < night. No fever in the morning. Profuse sweat. Abdominal pain. Heavyness of head. Cough < afternoon. > vomiting of mucus. Loose stool once in a day. Blood for widal +ve. Blood report attached herwith. Please suggest the Rx.

85 yr old male with vesicular skin lesion (First started on the back ,groin and inner thigh then slowly spread all over the body.the patient has the same kind of lesion on the tongue ,buccal mucosa , hard palate) for past 5 days. patient had fever with chills and rigor. patient complains of severe pain over the lesion . No complain of itching. patient complains of mild abdomen pain(over the right hypochondrium ,umbilical& lumbar region) for past 3 days. patient also complaints of not passing stools for past 3 days (but today morning he passed the stool).No complain of nausea ,vomiting, loose stool . No complain of sleep disturbance. kindly suggest what treatment to be done and how long and what investigation to do.Do we admit the patient in higher centre and treatment him as Inpatient or can we treatment him as general op patient?.what advise do we need to give to the patient.
