13 years male has itching in fingers and hand and other parts of body . If he rubs swelling will occurs .please rx and dx

6 yr old child comes to me with h/0 itching rashes since 7 days. Pt has taken becasule cap. N cetrizine since 5 days as advised by some local practitioner bt no response . Respected seniors Kindly give your expert opinion .how to manage?

Mother brought her 3 month female baby with these rash for 2 days , increasing in size . Baby is otherwise active, happy, comfortable, taking feeds well EXCEPT MILD FEVER(98.8F). What is the diagnosis??
