Unknown Insect bite with cellulitis

32 ye old male, started complaining of itching n pain at the site of lesions shown in 1st two pictures a week back. He took a tetanus shot the next day.. The itching subsided post that but there was swelling around the ankle. Around 2 days back there is pus oozing out as shown in the 3rd picture, & c/o tremendous pain & swelling at that site. So he started with antibiotic & painkillers. The last picture is the most recent one. What could be the diagnosis & please suggest a line of treatment? Note: he works on a construction site, where there is a possibility of muddy area.

He can be put on broad spectrum antibiotics like Augmentin 625mg tds, Tab. Chymoral Forte 1 tds, Cap. Becosule 1OD, NSAID for relief of pain, Tab. Allegra 180 mg once daily to combat any allergen. Locally Mupirocin oint . Elevation of Limb, Rule out Diabetes Mellitus

Carefully look at lesions Initially there are patches of sunburn on feet and has photodermititis Irritants induced itching Frequent itching or scrubbing created abrasions and breaching of epidermis invited infection landing in cellulitis Hence approach is Tab azithromycin or clindamycin Tab Levocetrizine5mg Antiinflamatory Once infection resolved shift on photodermititis adv change of footwears Adv moisturisers frequently and sunscreen lotions and protective from sun exposure

Itching due to some allergen has caused skin lesion and superadded bacterial infection. Topical application of cloben G twice a day. Advice to clean his legs after return from work.

Carbuncle Pus to be sent for bact . culture Rule out fungal infection Rule out actinomycosis Then treat with antiinflammatory analgesics Antibiotics etc.

Use corticosteroid with clindamycin300 mg.
