36 years old, unemployed,presented to infectious diseases department by his ophthalmologist for the follow-up of the treatment he has established for his mystery pathology.He had consulted his ophthalmologist because he saw less well and one of his friends found that "his eye was weird" In addition, he is in great shape, with no particular medical history. 1) What is the name of this pupillary deformity? 2) Which ophthalmic entity can it match? 3) In the context, which disease should be sought? 4) How is this disease treated?

Festooned pupil due to Posterior synechiae . This is case of iridocyclitis. Following diseases should be investigated for : 1. Sarcoidosis . 2 .Tuberculosis . 3 . Toxoplasmosis . 4. Herpes zoster . 5 . Syphilis. 6. Lyme disease . 7 . Wegener's granulomatosis . 8 . Leprosy . rx: * Rx of specific disease if any is found. * steroid with antibiotic e/drops. * atropine e/drops . * systemic steroid sos if not contraindicated.

Ankylosing spondylitis , rheumatoid arthritis , Ulcerative colitis & crohn's disease r also important diseases to be looked for .

Looks like a case of uveitis. Most likely anterior uveitis. Posterior synechie are present. So patient might had anterior uveitis in past. Festooned people is the term used for such pupil. Go for complete workup to find out the cause of uveitis. Rule out any systemic associations.

Chronic Iridocyclitis with Posterior synaechia causing Fastooned pupil . Investigate for systematic causes and treatment with cycloplegics and topical and systematic steroids.

FFestooned pupil. Ant Uveitis Chronic systemic infection /inflammation should b ruled out . Needs local/systemic steroids for maybe long duration

Clover leaf pupil due to posterior synechie most probably due to uveitis.
