The right answer is medullary sponge kidney. This is a Ds where there is cystic dilatation of multiple collecting ducts of medulla.This ultimately leads to medullary nephrocalcinosis which can lead to nephrolithiasis.The psychogenesis is unknown. the complications include renal infection,renal insufficiency and renal failure.

Bilateral renal calculi, in renal pelvis.

POSSIBLE NEPHROLITHIASIS

Multiple bilateral renal stones

medullary nephrocalcinosis

NEPHRO LITHESIS

ureteric colic due to stones

cervical bones spondilitis

Bilateral renal calculi

must be Lt Ureteric calculi as Hematuria& flank pain. manage as per size of calculi
