From CBC only apart from malaria No 1 in world can 100% exact cause of fever .. bro . Work up is there to find the cause . 1. Anemia is there , ask detail h/o diet , bleeding from GIT and GUT , PREVIOUS SX , BLOOD TRANSFUSION H/O . GO FOR B12 , IRON , FERRITIN , TIBC 2. WBC IS LOW CAN BE VIRAL LIKE DENGUE FOR THAT DENGUE NS1 AND IGM PROFILE IS THERE . LECOPENIA IS ALSO SEEN ENTERIC . FOR THAT TYPHIDOT IGM . THESE 2 ARE COMMON INFECTIVE CAUSE 9F LEUCOPENI. APART FROM THAT MANY OTHER ETIOLGIES ARE THERE . SO PLZ DO PROPER FEVER WORK CBC WITH PS FOR MP , URINE , SPUTUM , TYPHIDOT IGM , DENGUE PROFILE. THANKS

Anaemia is mild and not of much concern. Leukopenia may be due to viral infections, dengue, malaria, enteric fever (though less likely in 1-2 day's fever), HIV etc. Neutrophilia indicate acute bacterial infections. In the given case causes might be.... 1. Viral infection complicated secondarily with chest infections. Acute onset, cold, cough favour this. 2. Pneumonia secondary to viral infection. 3. Enteric fever. But it's less likely considering associated symptoms. Although leukopenia is a late feature of enteric fever. Lymphopenia here may be due to neutrophilia (relative lymphopenia) or true lymphopenia. However absolute count of lymphocytes can explain it. In given case my PD is VIRAL INFECTION WITH SECONDARY CHEST INFECTION. Management should be.... 1. Adequate hydration 2. PCM for fever 3. Amoxyclav for chest infection 4. Ambroxol and Levosalbutamol or Terbinafine based cough syrup. 5. CXR, Blood for Dengue NS1, Typhi dot, HIV.

It is simply viral fever. It is however seen that cont becomes so high or lowered. Anemia is also to be taken in record. Start cefaxime and Ofloxacin combination. Iron tonic Rabeprazole with domperidone combination. It will take 5 days to be normal.
