cranial nerve palsy.

40 yrs hypertensive lady c/o drooping of lid in left eye following headache & occasional vomiting for about 10 days. She has not taken any rx. She had similar illness in rt eye about 1 yr back which was cured by rx .dx & rx ? Thanks.

An acquired third nerve palsy It may be pupil-sparing or pupil -involving When there is complete ptosis and normal pupillary reaction associated with pain it is pupil-sparing .Since the patient is hypertensive it points toward it.

Differential diagnosis of cause of 3rd nerve play Presumed microvascular Trauma Compression from neoplasm Compression from aneurysm Diabetic Do MRI brain
