Lesion involving metaphysis: physis and epiphysis and probably extending to joint Metaphyseal periosteal reaction likely Infective pathology like acute osteomyelitis with joint sepsis the first possibility Usg evaluation to identify abscess if any and joint effusion Needle aspiration of involved bone and knee joint Gram stain and culture If positive plan arthrotomy and joint lavage and wash out of bone Can be done with same incision CBC esr If not sepsis,plan MRI evaluation
