45 year old female History of pain abdomen from 1 month Known-diabetic Fever with chills Ultrasound abdomen and CT scan abdomen contrast attached Splenic abscess Discuss the management?

USG and CT suggestive of splenic abscess with grade 1 fatty change of liver Splenic abscess is a rare entity mainly spreads due to haematogenous spread of sepsis intraabdominal or by direct spread from the sepsis like acute pancreatitis , acute cholecystitis with or without perforation. As clinically diagnosis is very difficult so prognosis also at time be ones unpredictable Diagnosed by USG or CT Treatment Higher antibiotic If abscess cavity unilocular or bilocular US guided aspiration Bilateral nephropathic changes to be evaluated if necessary by renal biopsy.

Fever is due to of bactermia primary treatment is high antibiotics with clindamycin. IF no response then go for spleenectomy which is standard treatment

Cause of fever might be because of splenic abscess Treat with higher antibiotic Do pigtail at splenic abscess N treat accordingly
