45yrs old female patient suffering from severe cough since 15, with frver and chills. Loss of apetite. Monteux test after 36hour. Plz suggest me the monteux report and diagnose..??

Don’t start ATT of the patient on the basis of Montoux test. Get an X-ray done of the patient. Take history of tb contact . Give symptomatic treatment as for now.

Sir.. i am giving her amoxyclave625mg as antibiotic, doxophylline 400mg, LTz-M, bt not improving.. n what do u think about the montoux..?? Is this positive..??

This can be viral pneumonia. Patient should be started with antibiotic with 3rd generation cephalosporin, Tamiflu for suspection of h1n1 virus, Maintainence of hydration and other symptomatic treatment Cxr to rule out type of pneumonia, Sputum for afb to be done also, H1n1 to rule out influenza like illness, Blood culture, Complete haemogram, Arterial blood gas, Urine routine and microscopy

NEEDS TO EVALUATE CLINICOPATHOLOGICALLY.. BLOOD CBC ESR URINE ROUTINE SPUTUM FOR ZN STAINING X-RAY CHEST BRONCHOSCOPY..SOS.. MEANWHILE TREAT AS C/O LRTI..

Needs further investigations and evaluation before starting ATT. CBC,XRay chest,sputum for AFB, ESR and CBNAAT. Symptomatic treatment till reports pour in.

Mantoux test positive but it is not diagnostic,Do Sputum for AFB, CBNAAT, CXR,ESR,ICTC,BSL to facilitate diagnosis

HRCT chest. Sputum for AFB repeat. Previous history of taking ATT. Or vacancies of BCG gives positive result. Treat like LRTI. Most of the time patient not give proper Sputum sample or only spiting in container.if patients not respond to the conventional treatment Go for brochoscopy. Broncheioscopic Aspirate or washing sent for examination AFB CBNNAT

Adverse reaction,(blistering & tubercles) of tuberculin skin test.

Do CXR, sputum for afb, CBNAAT, If negative thn and patient affordable then do ct chest (HRCT WITH CONTRAST) check report for mediastinal LN or any other lesion... then opt for bronchoscopy.... U will get diagnosis. Dont give amoxyclav until you have diagnosed tb in suspected cases... As it is anti TB.

Mx negative Go for xrzy If scan is nad then this pt should be taken as routine casual pt But before making any guess go for scan

Positive, but it however is not confirmation of Tb, cxr, and if in dilemma get CT chest

Ok sir.. bt what if chest xray wil b wnl then.. what does the montoux test positive mean..??
