46 yrs old lady presenting with a hard lump in upper outer quadrant of Left breast, increasing in size for last 6 months.on Clinical examination,lump is of size 8cm×6cm in UOQ of left breast , nontender, normal temp,hard in consistency,fixed to breast ,no fixation to skin , underlying Pectoral,Serratus muscles, axillary nodes not palpable. Right breast and axilla normal. FNAC shows Ductal Carcinoma.plz suggest treatment.

It's a very large lump T3 as per TNM classification She can be staged as locally advanced breast cancer Metastatic work up needs to be done such as PET CT SCAN Trucut biopsy with Histopathology, ER, PR and HER 2 Neu status needs to be investigated She is likely to benefit from neoadjuvant chemotherapy in consultation with oncologist followed by modified radical mastectomy.
