48/F, mesenteric mass.....15×10 cm.....outer surface bosselated, cut surface multinodular, firm grey white with areas of necrosis, hemorrhage.......

Shows biphasic morphology of clear/ epithelioid cells & spindle cells arranged in whorls, fascicles & nodular pattern, with minimal pleomorphism & areas of necrosis. Correlating site, histomorphology & ihc positivity for D2-40, Malignant Mesothelioma may be condidered.

As CD23 & D2-40 are positive,FDC sarcoma is a possibility.

This case was initially reported as D/D of MFH, Inflamm myofibroblast tumor and Gist.....another very reputed private lab on review and IHC called it as most compatible with follicular dendritic cell sarcoma!!!!!!! We are still in a fix........hv ordered regross, and Extended ihc panel...... the previous lab ihc slides are not avialable for review....... bur they did not do clusterin, CD 21, CD 35.....only CD 23 was positive(FCER 2) along with D2 40.. Melanoma, lymphoma, carcinoma,markers were neg......CD 117, DOG 1 neg....... FDC secreted protein (FDCSP), Serglycin, Claudin, CXCL 13 are some recent novel markers for FDC sarcoma.....we dont hv them......
