Metachromatic leukodystrophy

49 years old patient presented with a 5 days H/o progressive holocranial headache, sensation of thermal rising and disorientation. He has new onset lack of recognising relatives and a tendency to sleepiness. Chief Complaints Headache,Prosopagnosia History HTN Diagnosis WHAT IS YOUR DIAGNOSIS?

Dear Dr Vedan. This is not the picture of metachromatic Leukodystrophy. M.leukodystrophy is a genetic disorder, MRI picure is predominantly periventricular white matter involvement. . THIS IS GRAY AND WHITE MATTER DISEASE ,BOTH TEMPORAL ,PARIETAL ANF LEFT FRONTAL. This is multifocal leukoencephalopahy. Investigate further

Highly unlikely to be a metachromatic leukodystrophy with only 5 days history As Manorama mam already mentioned that mri is not typical for same. For such a short history one must evaluate for other etiology
