Arteriovenous malformation

30/f, 30 weeks pregnant, 3g2p, presented with seizures only, v/s normal, not full term, no features of toxemia or acc HT, still treated by gynecologist as a case of eclampsia!! later MRI done.. diagnosis, decision making & management plan is a matter of expertise.. presenting the MRI

it's a case of arteriovenous malformation... it's a complex congenital disease.. treatment is surgery or embolization or both. if presentation is only seizure, as in this case, one can start AED and observe.. if it bleeds, surgery is necessary.. in either case, embolization is first choice if feasible. this case with pregnancy is advised to pull till full term,on AED, cautions, and then elective caesarean followed by treatment for avm
