1st Cxray shows Rt lower zonal inhomogenous heziness and rt paracardiac rounded mass lesion/ scalloped diaphragm noted. Findings sugg of rt middle lobar consolidation with fissural effusion. Possibly associated consolidation and empyema. Needs broad spectrum antibiotics initially and send sputum and pleural fluid culture and sensitivity test. Metro, Amikacin and amoxyclav would be helpful initially.
