The submental space is a fascial space of the head and neck (sometimes also termed fascial spaces or tissue spaces). It is a potential space located between the mylohyoid muscle superiorly, the platysma muscle inferiorly, under the chin in the midline... Signs and symptoms of a severe submental abscess include a firm swelling below the chin and dysphagia (difficulty swallowing). Treatment is by surgical incision and drainage, with the incision running transversely in a skin crease behind the chin.

It is sub mental abscess occupying the fascial space of the head neck, a potential space in between mylohyoid muscle above and platysma downward The abscess in this space requires incision transverse along the skin margin and to explore the fascial tissue space to drain the pus under GA. Antibiotic. Amoxiclav syrup 5ml TDS for 7days Tab serratiopeptidase 5 mg tds for 7days Analgesic as per requirement.

D/d Submental space infection Lymphadenitis Tubercular lymphadenitis. Advice opg, fnac Give supportive treatment to the child Look for any periapical pathalogy if present. Incision and drainage under ga if respiratory obstruction is present. Hospitalization if necessary Antibiotics and analgesics to be started. Also advice chest xray

M 24 c/o General weaknesses & chr fever with painful cervical lymph nodes for 1&1/2 years. No family history, patient had contact with one, sukha tb ie pleurisy or effusion. Patient presented with B/L multiple matted cervical lymph nodes with discharging sinus, painful, adherent to skin, some non-suppurative LN are mobile, non-tender on supraclavicular, submandibular,, Rt axillary areas. O/E Pulse 106bpm, regular, at present afebrile, built below average, poor nutrition, BP 100/60, other general survey unremarkable. RS normal, CVS S1S2, Abd semisoft, no ascietes, diffuse palpable LN No neurodeficit. INV Slight anaemic otherwise CBC Normal, HBsAg, Anti HCV negative, LFT Normal, Sugar F 94,PP 128,BT 1'45" CT 5'15" TSH 0.8,Urea 22,Creat 0.9,ICTC N/R, Sputum AFB -versicolor, Sodium 133.8,Potassium 4.03,ECG Sinus tachycardia Wedge biopsy of Cervical LN showed Nacroinflammatory tissue. AFB N/F. USG Abd multiple abd LN, peripancreatic, mesenteric, pre & para aortic & iliac. Prostetic cyst with wall calcification, B/L mild raised renal echogenicity USG neck multiple enlarged LN in level ll, lll, lV -large one 17mm in Lt side of level ll, B/L lobes of thyroid normal. What would be the diagnosis? What would be the line of management?

Patient by male ,50 years ,c/o left side of face swelling, extending anteriorly up to angle of mandible ,posteriorly beyond the sternoglydomastoid muscle ,above up to tragus ,below up to middle of above said muscle. Circular swelling ,hard in consistency,, movements well restricted ,on clenching it is more prominent .On flexion of neck swelling more prominent. No significant lymph nodes. oral cavity normal. Diabetic .On treatment under good control. Discuss the Differential diagnosis .
