trigeminal neuralgia

50 years male, c/o Right sided facial pain originating from right side of gum, passsing to head followed by dizziness for 2 days. Head is also associated. On further query, he told he was found hypertensive but took no medicine. He toabcco chewer. O/E he was found tachy and bp 160/90. Pupil b/l normal. Lungs and hearts are normal. No weakness appreciated. On this basis what can be differentials? Rational test and how can we manage?

Most probably- it is a case of trigeminal neuralgia. Advised MRI brain Tab Oxcarbamazine,300 mg O.D for 2 months Tab Gabapentin 300 mg + methyl cobalamin 500 mg O.D Naprosyn 500 mg S.O.S

May be because am an oral surgeon..I would suggest to rule out any dental or oral etiology before proceeding with TN protocols. Especially when he is a Tobacco chewer. An intra-oral examination will suffice wrt to the right area ,gums and sulcus region. If needed with radiographs.. thanks and good luck doc.
