51 year old female presents with red rash on bilateral lower extremities. Pt also presented with swollen feet/ankles with +4 putting edema. Pt states the swelling gets worse progressively throughout the day and is the worst at night but improves upon waking. Pt states she is tired most of the time and has shortness of breath upon exertion. Pt has a history of a hypertensive crisis that required a hospitalization. Pt takes Losartan and Amplodipine daily to control her hypertension. What is your diagnosis?

11 year old patient comes to the office due to malaise, nausea (1 vomit which she recalls had blood in it), sore throat, cough and the appearance of a maculopapular rash in anterior and posterior thorax which disappears as you press it, also it’s not palpable. As you can see from the images she has conjunctivitis, and the tonsils were frankly suppurative. She doesn’t recall having fever but she did had a Acetaminophen pill. During the consultation she had 37.6°C. I could also appreciate painful nodules on her anterior neck as well as little enanthema on the “hard palate”. No sore joints or muscles. (3rd image is anterior thorax)
