Respected Curofyians, please help me to solve this case. A female, 18 years, overweight. C/o difficulty in movements of right upper limb 2 years ago when she was studying 10th standard. Occasional tremors were observed in the hand initially. She could write. Lost ability to lift weight and fine actions gradually. Was taken to neurologist. All relevant investigation done Diagnosed to be Dystonia of right upper limb. She is feeling and mother is also telling she has become extremely slow in her actions. She has become mentally also slow and learns very slow. Because of which she has been failed who was among toppers​earlier. Now on examination Power good. Tone reduced to 3 No atrophy. No tremors Reflexes good and intact!!! Appetite good. Lifestyle good from beginning Menstrual history no problem

About half the people with Wilson's disease have neurological or psychiatric symptoms. Most initially have mild cognitive deterioration and clumsiness, as well as changes in behavior. Specific neurological symptoms usually then follow, often in the form of parkinsonism (cogwheel rigidity, bradykinesia or slowed movements and a lack of balance are the most common parkinsonian features[7]) with or without a typical hand tremor, masked facial expressions, slurred speech, ataxia (lack of coordination) or dystonia (twisting and repetitive movements of part of the body). Seizures and migraine appear to be more common in Wilson's disease.[5] A characteristic tremor described as "wing-beating tremor" is encountered in many people with Wilson's; this is absent at rest but can be provoked by extending the arms.[8] Cognition can also be affected in Wilson's disease. This comes in two, not mutually exclusive, categories: frontal lobe disorder (may present as impulsivity, impaired judgement, promiscuity, apathy and executive dysfunction with poor planning and decision making) and subcortical dementia (may present as slow thinking, memory loss and executive dysfunction, without signs of aphasia, apraxia or agnosia). It is suggested that these cognitive involvements are related and closely linked to psychiatric manifestations of the disease.[7] Psychiatric problems due to Wilson's disease may include behavioral changes, depression, anxiety disorders, and psychosis.[5]Psychiatric symptoms are commonly seen in conjunction with neurological symptoms and are rarely manifested on their own. These symptoms are often poorly defined and can sometimes be attributed to other causes. Because of this, diagnosis of Wilson's disease is rarely made when only psychiatric symptoms are present.[7]

Vikarnam akushlo na jihiyat kadachana l Na hi sarvavikaranam namtoasti dhruva sthiti ll. Ch. Su.18/44. There is a theory in ayurved name AVRAN SHIDHANT. Acharya charaka and susruta both mention it. In kapha aavruta vata and other prana vata avrata vyana vata have similar symptoms. Ch. Chi. 28/ 202. In this case history we can see some disturbances of functions of vata and kapha. 1 So can we think about aavarna.? (Experts opinion requires) 2 can we think about chikitsa of kapha and vata.? Acharya also give a beautiful management- 1. urdhajatru karma ( nasya) 2. vamana 3. vata syamaka ausadhi ( all are mention by previous respected doctors.)

I think in this case verachanam is helpful, abyang with mahavishagarbha+Bala tail, nasya with anu tail or badam tail, yoga of sameerpanag rasa +khanjnikari rasa +giloy satva+trayodashang guggulu +ashwagandha churna should be taken with honey bd, kraunch pak 1 tsf with luke warm milk, bramhi ghrita, avipattikar churna 2 tsf with luke warm water hs

Along with these medicines advised by Himanshu sir, We can add Anu Tailam Pratimarsha Nasya Shirodhara with Takra (Takra Dhara) Yoga and Pranayam on routine basis Educate about Pathya-Apathya and Nidana Parivarjana
