52y/o female patient with blackish discoloration of lower and upper extremities since more than one year a/w itching(more at night as per patient).could not ellicit scabies .. what is the dx and mx?

Ichthyosis ? Photodermatitis Moisturizer or emollient for rehydration of skin. Clean area with diluted betadine lotion apply calamine lotion than apply mild steroid cream. Sulphur oint 2% locally Hs. PABA cream locally during exposure to sunlight. Multivitamins and antioxidants orally. Improve general health and personal hygiene. Ketoconazole soap,lotion and soap. Levocet for itching. Vit aAnd E orally and topically. PCM+ibu to reduce pain and redness. Green lefy vegetable.

Icthyosis Vulgaris. Soak with tab water with gentle wiping of skin followed by emmulient. Topical Kartolytic agents: Urea(10-20%), Propylene glycol (40-60%), Salicylic acid 6%. Tab Levocetrizine5 mg BD×3 weeks.

It's a case of contact dermatitis.

opinion changes when a pt is seen actually , photographs not fully reliable- the picture looks like generalised exfoliative dermatitis - causes are many - even one has to keep in mind presence of internal malignancy - for symptomatic quick relief - Mix Triamcinolone 40 mg inj in half cup of coconut , apply twice or thrice daily oil for 2-3 week & gradually reduce the frequency of this application - an d keep on applying coconut oil or liquid paraffin -this is extremely good & cheapest , one can add multivitm vit d, etc..this will avoid additional taking of antihistaminic ie cetrizine- calamine is not at all preferable , it self causes irritation unless mixed with oil ie make it a liniment, Sulfur may cause secondary dermatitis on the existing dermatitis
