56yr male having known case of COPD on ICD has got sudden swelling of whole body and diagnosed as a subcutaneous surgical emphysema kindly share your views on Rx and experience

1st image showing facial puffiness. Clinical history sugg of COPD with ICD. Needs to manipulate ICD if air leak is from side hole. See under water seal is maintain. Avoid bipap as it may worsen the situation if source of surgical emphysema is from different places. Subcutaneous skin nicks may be helpful in draining the air. Give nasal oxygen support to maintain spo2. 2nd image is of a female (perhaps different patient) showing Pulmonary plethora and ? PAH.

Patient on ICD - but X- ray doesn't show the ICD or need for ICD .and if present should be removed after a fresh X- ray. Oxygen support with mask to maintain SPO2 is indicated along with subcutaneous skin small incisions to help in draining the air . May require hydrocortisone to prevent any laryngeal oedema

X-ray has no evidence of pneumothorax or surgical emphysema. Cardiomegaly seen Surgical emphysema cannot produce edema of the whole body ? Drug allergy, ? CCF, Renal causes , fluid overload

Xray chest shows Cardiomegaly Lt lower zone haziness obscured cp angle Prominent bronchovascular markings bilateral I couldn't locate ICD Face of pt suggest puffiness H/o surgical emphsema Pt should receive niv to keep 02 saturation Workout for obscured lt cp angle And adv 2decho and ecg to study cardiomegaly

On ICD now? Chances of REPE is there..continue high flow oxygen.
