Atopic dermatitis Associated secondary bacterial infection . However, vascular insufficiency due to diabetic induced PVD and varicose veins need to be ruled out. Get a colour doppler study of lower limb vessels. Achieve strict glycemic control with insulin. In case of PVD - add tab cilostazol 100 mg B.D to improve blood supply for 2- 3 months. Oral antibiotics, topical moisturizers . Another D/D is autonomycosis and madura foot needs to be ruled out by a biopsy
