58 year lady comments on her toes. She states that during the day her toes gets red, hot toes but no swelling but pain. This redness occurs mainly in the evening but also during the day but to a lesser extent. The rest of her feet feel cold with a noticeable colour difference She has had previous episodes last year of this but it went away lasting a few months. On examination she has no sensory loss in either feet, full motor function and all her pulses are palpable. Her soles of her feet have numerous callouses,. her past medical history include diabetes for 6 years, and general arthritis in her knees, back and hands. Any idea?
Its because she have used to long nails... Now its trimmed and due to tight athlete footwear its not adjusting, advice to wait few days..... Advice foot bath with Sphatika Bhasma..... 2nd Pic Charma Keelaka.... Advice for surgical management....
@Dr. Nisar Patel , Thank you Doctor for requesting my opinion. The toes are swollen and appear inflamed,plus if you observe carefully, you will see that both the big toes (thumb) are bending outwards laterally... this happens when the foot is compressed continuously by either wearing tight shoes, a size smaller or by narrow pointed shoes. @Dr. Kamal Singh pls enquire with the patient if she has been wearing narrow pointed shoes, in which case she will have to give up that narrow footwear and wear chappals or broad footwear. Narrow pointed footwear is a big fashion faux pas. Human feet are not naturally tapered at toes but are broad. However if this has not been caused by footwear, then do rule out early onset of Cellulitis. From the picture, it is seen that there is a corn on the sole not a callus which are if present, then where on the foot. For the time being I would suggest Homeopathic medicine BELLADONNA 200/tds/one week. When this acute inflammation subsides, we can begin proper treatment for corns and/or callus.

Check for varicose veins . Its better get ABI study done to know the the status of blood circulation in LE SEEMS TO HAVE SOME OBSTRUCTION IN BLOOD FLOW . . WHENEVER THIS HAPPENS ASK HER TO LIE DOWN WITH LEGS RAISED ABOVE LEVEL OF HEART AND SEE IF REDNESS DISAPPEARS .

It may be due to hypertension which is always present in diabetics since it is a chronic case it might have created a vascular damage which is likely to cause gangrene of the affected part. 68 years old diabetic male comes to clinic with pain in the left foot and difficulty walking since two months. He also have HTN and Dyslipidemia. But all the three conditions are fairly controlled.
