Palpate bimanually , if palpable from inside mouth & outside as well, than it is submandibular sialadenitis, if not than may be lymphnode, get an ultrasound done to confirm Get FNAC done Give non quinolone antibiotic for 5 days, subsides ok, not-if sialadenitis-removal of submandibular gland If lymph node-remove & get H/P done

Many conditions come as d/d. Starting from simple abscess, alveolar abscess siloadenitis, sebaceous cyst come on the way. FNAB is very helpful in diagnosis and proceed accordingly.
