60 years male Itching , pricking sensation since 1 month, No marital and extramarital contact since 10 years . What is the diagnosis in this age ? Management ?

Good answers doctors , Here my impression is SCLEROTIC WHITE RING at the tip of the prepuce with purple white plaques at the edges of the prepuce and reddening of the glans penis more favour to BALANITIS XEROTICA OBLITERANS(BXO) or Penile Lichen Sclerosus . Treatment : 1) Start with Potent topical steroids like CLOBETASOL and gentamicin cream. Tappering with mild steroids . 2) Symptomatic treatment is advisable . 3) CIRCUMCISION surgery with glans resurfacing with grafting .

There seems to be acquired phymosis if so the lesion is Lichen sclerosis atrophicus (LSA) Circmcission and hpe of excised stuff.

If no phymosis consider genital , vitiligo. R/O DIABETES AND FOR DD SAKE CANDIDIADIS. LSA IS PREFFERED FIAGNOSIS SUBJECT TO CONDITION LAYED.

Vitiligo

Rule out diabetes. Antifungal cream.Local steroid cream if irritation severe. Can take additional help from a senior Homeopath

Leucoplakia common in tip of penis itching due to secondary infection

Smegma clear with cotton dip in topid water with phimosis adv circumcision

Diabetes with leukoplakia due to acquired phimosis. Candidiasis

Phimosis due to moniliasis , DM if present , Must be treated . Malignancy to be ruled out. Secondary infection to be treated with appropriate antibiotic , followed by circumscision .

Lichen sclerosus of penis A few homeopathic medicines are effective to relieve it. Though INDIVIDUAL medicine is the best still THIOSINAMINUM 3, in repeated doses is helpful
