60 years old male came with c/o mild itching scaly lesions which started 2 months back with small papular lesions on bilateral upper and lower limbs, scalp and back(extensor area) . Itching increases with warmth. Kindly diagnose and advise management.

Dr.Rahman, Persistent lesions of PSORIASIS in the LUMBO SACRAL REGION with pigmentation due to chronicity( common site) legs and silvery scales more favour to RECALCITRANT PSORIASIS. psoriatic arthritis is common in these patients. TREATMENT: 1) Salicylic acid 12% on lesions morning Moisturiser with vit E in night 2) CALCIPOTRIOL on recalcitrant lesions 3) Resistant to conventional therapy Advise to go for Combination of INFLIXIMAB and HYDROXYUREA.

Plaque psoriasis.Topically halovate s n moisturex mix and apply.tab atatrx 25mg at night.after lft and rft pt can advised for methotrexate 7.5mg weekly with folic acid combo.avoid using oral steroids which may complicate the case pustular psoriasis.
