60yr/F with elevated renal parameters urea - 60 creat - 2.2 on Olmesartan medoxomil 20mg OD , T.nefrosave OD complaining of on/off decreased micturition releaved by taking T.nefrosave Not a diabetic SHT on Olmesartan for 5 yrs Dyslipidemic on Atorvas 20mg Is this safe to continue ACE/ARB in elevated renal parameters? She can't afford Olmesart as it costs 10/tablet I suggested her to start Amlodipine 10mg OD Is this suggestable?
Ace inhibitors & ARBs should not be used in patients with baseline hyperkalemia. So, you can start with Amlodipine + atenolol combination.
Prefer betablokers Alfa blockers CCB. Avoid ACE ARB
Try to use of Vasa swarasa or churna with Ghee , vasaghruta. I think here we should consider the Raktapitta samprapti (tiryakgata).

A case of diabetic male aged 43 years has not much problem except slight whitish discharge in 1st morning sample. Diabetic of 9 years duration, presently taking glyree 2mg BF,glycinorm GP2 in lunch and glycinorm GP1 in dinner. Also taking Telma 40 and Tonact 10.Diabeic diet control is good and daily 30 minutes walks /jogging. Reports show F 140 HBA1C 9.3.,PP >220. RE urine normal except 3+ sugar. Hb 12.6gm% All other test are normal including RFT, serum calcium total proteins. PSA is normal. He had recurrent attack of haemoptysis 5 years back for which underwent thrice bronkoscopic procedures in a reputed hospital in Mumbai, though xray was normal had been diagnosed as bronchiectasis. Also received full course of ATD.AT present no problem from that side but ESR is 32.What should be the line of treatment now to control diabetes.

seven yrs Mohomedan girl two month back presented with irregular fever with cough and cold with Pain both inguinal region with inability to walk properly.At that time all her peripheral pulses was normal except lean and thin built with sick look and diffuse alopecia and inguinal lymphadenopathy which were sotty and non tender. At that time her ESR was 65 CRP 20 Hb 10% Pain over inguinal region was continuing during that period mantoux was 8 mm CXR was normal. Pl did not came for follow up for one month .At present She developed blckening of acral parts with coldness but no pain for 15 days anf suffered a tonic clinic conclusion 7 days back followed by paresis of Rt upper limbs and left sided facial palsy evident from the photograph. Chest full of cross Gc Poor pulse 119 per Min low volume dorsalis Pedis both sides not palpable. Pl suggest provisional diagnosis and d/d and management
