65 yr old Male patient having c/o vertigo and weakness with history of Melena since last 7-8 days...BT is already given..what the next plan of Rx??

Needs urgent OGD scopy once hb is more than 8... Most common causes are peptic ulcer bleed, variceal bleed, mallory wiess tear, duelofoys lesion... Start on IV PPi infusion at 8mg/hr with IV fluids.. Do Hb every 8 hourly and correct anemia if it's there..

Peptic ulcer. Gastic carcinoma. Do endoscopy foirst then follow the treatment.

Send stool for occult blood. Do PR. USG abdomen You may seek advise from gastroenterologist and plan UGD scopy. However patient needs admission.

Upper GI tract bleeding, endoscopy advice

have to find the cause of melena.. Any other investigation done?? LFT, Sr electrolyte, Colonoscopy or endoscopy?

Upper GI endoscopy and USG abdomen. As platelets were 1.8 lakh. Hence less likely to have cirrhosis. For this patient peptic ulcer and gastric malignancy are two common possible diagnosis.
