20 years history is very important. If history is to be considered malignancy is unlikely .Could be a benign tumour or a malignant transformation may have occurred in an otherwise benign tumour. F.N.A.C and CECT scan will help us in arriving at a diagnosis. D/D 1.Mixed Parotid tumour. 2 Ameloblastoma 3.Lipoma 4.Fibromyxoma

Such a huge tumour! God save the patient and his attendants.Long h/o 20 years.It is likely to be a benign tumour--Ameloblastoma. Required:-OPG,CT, MRI FNAC followed by surgery.

Oh My God!!!!! Where is the OPG???? Immediate intervention needed here to check out Ameloblastoma!! Please follow up the case with MRI or ct too. Plan surgery after confirmation. Histopathology too advised!

Patient's condition, general appearance is not so better that one can skip some malignant transformation here. CT immediately with HPE. If unavailable, go for OPG at least. It's shocking how patient has ignored it since so many years! Counsel not only him, his family members too so that this Ignorance is not inherited further!

A Pleomorphic parotid tumor causing pressure symptoms?Or Ameiloblastoma? Please do Ct scan.Do biopsy and plan for radical surgery.

unlikely to be parotid they don't extend to chin more likely to be mandibular origin. ameloblastoma.

@All...This is surely malignant...His Lips are dark/burnt ...beedi smoker..&sclera is not clean... He has senile arcus...cataract due to beedi smoke...

Plan for CECT base of skull to thoracic inlet A tru cut biopsy for HPE Looks like a parotid tumor but 20 yr is a long history.
