65 yrs male c/o fever,breathlessness for 2-3 days non diabetic non htn Oe spo2 94 %febrile hr 128/min bp 130/70
Lobulated opacity right midzone ,? Mass ,? Pneumonia. Left Synpneumonic pleural effusion Cardiomegaly ? COVID pneumonia ,? Mass Suggest CT thorax, ECHO ,left pleural aspirate analysis for AFB and CBNATT
Cardiomegaly LVH. Rt mid zonal inhomogenous opacities seen. Sugg of pneumonic consolidation. Adv CECT thorax to rule out organising pneumonia/ neoplastic lesion.

A 72 y/o male with a history of COPD and recent pneumonia was treated with oral prednisone 40 mg daily, antibiotics, and a fluticasone-salmeterol inhaler. One week during treatment, the patient developed painful oral lesions. examination revealed many white-tan plaques on the lower lips, tongue, and posterior aspect of the oropharynx. Lesions do not respond to nystatin oral suspension. What is diagnosis and management for the case?

64/F, non DM, non HTN, presented with Chest Pain, SOB, Weakness and Low grade Fever for 3-4 weeks. ASO- 387, CRP-57.38, Spirometry- Normal study, Sputum AFB- NF for 2days. Echocardiogram- EF67%, no RWMA, IVS & I AS intact, Valves normal. CXR, CT Thorax, ECG, CBC, Electrolytes attached.
